For Corporation
a. Articles of Incorporation and By-Laws with SEC Registration Cert.
b. Cert. of Registration of Business Name from Dept. of Trade and Industry (DTI)
c. Notarized Corporate Secretary's Certificate on:
1. Board Resolution designating Maxbank as depository bank
2. Duly elected incumbent officers of the corporation
3. Board resolution designating corporate officers authorized to act as account signatory/ies (designations must be consistent
with the provisions of the corporate by-laws)
d. Presentation of at least two (2) acceptable identification and recent photograph of the authorized corporate officers from
